The EYFS Framework (2021) states there should be a progress check carried out at age 2…
“When a child is aged between two and three, practitioners must review their progress, and provide parents and/or carers with a short written summary of their child’s development in the prime areas. This progress check must identify the child’s strengths, and any areas where the child’s progress is less than expected.”

TRANSITIONS - Changing/Moving Settings - EYFS Handover Pack
This pack is intended for use by practitioners in pre-school EYFS settings (Nursery, Pre-School, Childminders) to compile information to be sent to a child’s new key worker when moving to a new setting.
Elements of this pack could also be used by settings who would like to send home a pack for children to fill in with their families before starting.

EYFS FRAMEWORK - Child Record Forms
Child record form which comprises 2 A4 pages. The first page covers ‘Contact Details’ and the second ‘Additional Information’. If you would like to adapt the forms, simply choose the editable docx file.
The EYFS Framework (2021) states…
“Providers must record the following information for each child in their care: full name; date of birth; name and address of every parent and/or carer who is known to the provider (and information about any other person who has parental responsibility for the child); which parent(s) and/or carer(s) the child normally lives with; and emergency contact details for parents and/or carers.”

SAFEGUARDING AND WELFARE - Risk-Benefit Assessment Forms
These forms can be used to note the risks and benefits for activities that take place within a setting or on trips/days out.
As can be seen in the example on the first page, there is also a section to think about how the children might be encouraged to think and talk about the risks.
